Responsibilities

• Provide mental health counseling to individuals, groups, and families Conduct initial assessments.
• Provide crisis intervention as needed.
• Assure continued update of all case files and other records in strict compliance with policy and CMS regulations.
• Coordinates treatment planning.
• Assists with referral development and community education.

Requirements

• Master’s Degree in Social Work or Counseling and have an active Texas licensure as an LCSW or LPC or be license eligible.
• A minimum of two years of related work experience highly desired Experience providing group and individual therapy to senior adults in an outpatient setting.
• Experience facilitating patients' access to community resources.
• Demonstrate effective problem-solving skills and display proficient oral/written communication skills.
