Monday–Friday, 8:30 AM – 5:00 PM

As a hospice CNA, you will provide the hands-on personal care that helps patients feel comfortable, clean, and cared for wherever they call home, whether that's a private residence, assisted living, or a care facility. You'll work closely with a dedicated RN Case Manager as a valued member of our interdisciplinary care team, in a supportive setting with clear expectations and reasonable caseloads. Responsibilities:

Provide personal care with activities of daily living, including bathing, dressing, grooming,

toileting, and skin care

Assist
with lifting, transferring, positioning, range of motion, exercise, and ambulation

Assist
with dining and nutrition

Review and follow each patient's plan of care under the direction of the RN Case Manager

Provide comfort and companionship to patients and support to their families

Maintain electronic medical records

Qualifications:

Active Certified Nursing Assistant (CNA) certification (required)

Healthcare
caregiving experience in home care,
assisted
living/memory care, skilled nursing, hospital, or hospice
a plus

Current CPR certification (preferred)

Ability to work in an electronic medical record system

Strong communication
skills, including written, verbal, and active listening

Excellent customer service, problem-solving, and time management skills

Compassion, patience, and the ability to handle emotional situations with care

Strong understanding of professional boundaries

Ability to lift 50 lbs. and good understanding of body mechanics

Valid driver's license and current auto insurance (required)
