Responsibilities

• Accompany each resident on all medical visits.
• Stay with resident(s) until visit is complete.3.
• Observe the following guidelines at each visit: a.
• Performs other duties as required and assigned.

Requirements

• Physically able to help ambulatory and non-ambulatory residents transfer to and from wheelchair to vehicle.
• Possess a valid state driver’s license.
• Qualified as a Certified Nurse Assistant (C.N.A.).
• Clean driving record.

Responsibilities 1. Assure the safety of each resident when transporting to and from the doctor’s office and/or hospital.2.

Sign in resident upon entrance. b. Present resident’s documents to office personnel.4. Before leaving doctor’s office, please observe the following: a. Obtain resident’s documents. b. Obtain any new prescriptions. c. Check to make sure doctor has written on the doctor’s order form. d. If follow-up appointment necessary, obtain appointment cards.5. After returning to facility, report to RCM and be prepared to answer questions concerning visit.6.
